Under Minnesota law you have a right to:

- Expect that a therapist has met the minimal qualifications of training and experience required by state law;

- Examine public records maintained by the Board of Marriage and Family Therapy which contains the credentials of a therapist;

- Obtain a copy fo the Code of Ethics from the State Register and Public Documents Division, Department of Administration, 117 University Ave, St. Paul, MN 55115;

- Report complaints to the Board of Marriage and Family Therapy, (612) 617-2220, University Park Plaza Building, 2829 University Ave. SE, ste 330, Minneapolis, MN 55414;

- Be informed of the cost of professional services before receiving the services;

- Privacy as defined by rule and law;

- Be free from being the object of discrimination on the basis of race, religion, gender, or other unlawful category while receiving services;

- Have access to their records as provided in Minnesota Statutes, section 144.292; and

- Be free from exploitation for the benefit or advantage of a therapist.

You Also Have the Right to:

- Ask questions at any time

- Refuse to give any information you choose not to give

- Receive respectful treatment

- Have a safe setting, free from physical, sexual, and emotional abuse

Therapy Overview.  Therapy varies depending on the therapist, the client, and the client's particular situations and goals.  In order for therapy to have the best outcome, you will likely have to invest energy in the process and work actively on things we talk about during our sessions.

Therapy can have benefits and risks.  The risks may include experiencing uncomfortable feelings like sadness, guilt, anger, anxiety, or frustration when discussing aspects of your life or relationships.  Psychotherapy can have benefits that include better relationships, solutions to specific problems, increased life satisfaction, improved physical health, and significant reductions in feelings of distress.  However, it is impossible to predict or guarantee what you will experience.

Confidentiality and Data Privacy Policy (HIPAA).  This notice describes how medical information about you may be used and disclosed and how you can get access to this information.  Please review this and all other policies carefully.  I am committed to protecting my clients' privacy and confidentiality.  The U.S. Department of Health and Human Services (HHS) issued the Privacy Rule to implement the requirement of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).  The Privacy Rule standards address the use and disclosure of individuals' health information.  I must protect your "protected health information" that I have created or received about your past, present, or future health conditions, health services I provide to you, or payment for your health care.

Payment.  Payment of session fee is due at the time of appointment.  Payment may be made by cash, check, or credit/debit card, including cards associated with Health Savings Accounts.  Empowered Relationships uses Square to take payments in session, with card present.   Empowered Relationships uses Square as its payment processor to charge the card on file for late cancellations, missed appointments, and as directed by the Client to pay for sessions. 

Contacting Ania.  I may not be immediately available by phone because I do not answer the phone while I am in session with clients.  Feel free to leave a voicemail, and I will get back to you as soon as possible (usually within a few hours and generally within 24 hours Monday through Friday).  If it's an emergency, call 911.

Public Settings.  If I happen to see you in public outside of the therapy setting, I will only acknowledge you if you greet or acknowledge me first.  This is done to respect your confidentiality.

Other Fees.  Session fees are as stated on the Consent for Therapy Agreement.  Other services as may be necessary are billed at the following rates:

Report and letter writing:                  $30 per 15 minutes 

Phone calls beyond 10 minutes:         $30 per 15 minutes

Speaking fee:                                    $175 per hour

Consulting fee                                   $175 per hour

Legal related matters*                      $300 per hour

*Note: My services are not to be used for testimony, custody disputes, disability or any other form of court evaluations.  I am happy to refer you to other providers in the area who provide these services should you require any court evaluation or testimony.  Should I be subpoenaed or mandated by the courts to testify, you will be required to pay all fees, in advance, associated with the writing of case summaries, other reports, consultation with attorneys, consultation with mental health professionals, review of other records, and any other preparation.  The client will also need to pay all other costs associated with the court time, including travel time, parking, and meals.  All fees must be paid prior to the date of testimony.  These fees apply to any court-related or legal-related work regardless of whether testimony is required.  Any legal fees are outside of what is considered a mental health care services.

Social Media Policy. Empowered Relationships is on various social media websites as a way to market the services offered.  To protect your confidentiality Empowered Relationships encourages you to consider the public nature of social media before liking, fanning, or following our social media postings.  Messaging on Social Media sites such as Facebook, Twitter, Google+, or LinkedIn is not secure.  It could compromise your confidentiality to use wall postings, @replies, or other means of engaging with Empowered Relationships or Ania Scanlan online if we have an already established client/therapist relationship.  Please ask if you have any questions.

Non-secure media transmission and email.  Electronic communication (email and texting) is a commonly used way of exchanging information.  However, there is no guarantee that this form of communication is secure.  Empowered Relationships cannot ensure the security or privacy of the information exchanged.  Email is not an appropriate means for communicating about your therapy or about a mental health emergency. 

Ending Therapy.  You may discontinue or terminate therapy at any time.  Though this is not required when you discontinue therapy, I do ask that we have a final session to ensure appropriate closure.  I may discontinue therapy if the financial conditions noted in the Consent to Therapy Agreement are not met, or if a transfer to another therapist is recommended.
